First of all, when you arrive in Medellín, please manage your expectations. You're not instantly a god if you come from the 'first world', having that attitude will do more harm than good.

Okay, let's start.

If you're staying short-term: in most cases, forget it. Most non-trashy girls won't have a fling with you, you're seen as ephemeral (if you like trashy, you do you). Many gringos don't like to hear this, so they get frustrated when faced with reality, and they decide to 'pay for play' instead.

You have to differentiate yourself from the hordes of horny gringos that travel here for the same thing.

Your best chance is to speak some Spanish, learn to dance some salsa and reggaeton, dress well (no flip flops or shorts), and hit the party areas. I've seen girls have short-lived flings with foreigners, but it is uncommon. Don't expect a prevalent hookup culture, especially with a gringo; it's not really a thing here unless you're dealing with trashy women.

Instead of trying to take a girl to your place the first time you meet her, aim to secure a second date and get her IG or number. Most non-trashy Paisa girls like to take things slowly. You're fighting against a pretty strong stigma here: Women don't like 'sexpats', so acting as one won't help you. Flashing your money excessively will just make you look lame.

Your social skills matter a lot. Good-looking foreigners fumble Paisa girls all the time because of their weak game, looks won't take you to the finish line. Also, keep in mind that you're not magically more attractive just for being a gringo/foreigner. If you don't have game in your home country, you'll probably end up with some dubious girls who might give you scopolamine.

Avoid dating apps like Tinder, especially if you're not familiar with the language/customs. If you are familiar, you can vet the girls on that app, but do so at your own risk. Remember, if it looks too good to be true, you're most likely in danger. No respectable girl will bring another 'friend' to your place on the first date for some 'fun'. Please use some common sense.

It all boils down to your own 'game'/social abilities.

Safety tip: Live in an apartment with a doorman and tip him/her regularly. The doorman is your ally and could save your life if you're dumb enough to bring dubious guests to your place on a first date and you get scoped.
